Body Donation Packets

To Pre-register

If you are completing the form for yourself, please click on the "Living Donor Form" to the left. 

This form must still be printed and mailed to FACTS. Upon acceptance, FACTS will send you an acceptance letter and donor card.

To Donate a Loved-one

If you are the legal next-of-kin of a decedent and you are interested in donating your loved-one to our program, please complete the "Next-of-Kin Form".

For an expedited decision, the signed form can be scanned and emailed to FACTS. Original signed copies must eventually be sent in the mail for our records. FACTS reviews paperwork M-F. A form emailed over the weekend will be reviewed on the following Monday.
