Pick Up Request Form to Visit a Residential Camper During Camp

Please let us know your plans to visit a camp participant from the Junior Summer Math Camp - Residential (JSMCR) or the Honors Summer Math Camp (HSMC) during their stay at the Mathworks program. This is not the travel plans for your student to arrive / depart. For the safety of all participants and to ensure the participant is ready to depart, we ask you to complete this form. You will be notified by Mathworks staff when the request is received and proper individuals are notified.

For the safety of all participants, this form must be complete and approved prior to picking up your camp participant from the JSMCR or HSMC programs.
