2. Proof of U.S. Citizenship

  • Original and one B&W photocopy submitted with application
  • Original will be returned to applicant via USPS by U.S. Department of State. Separate from the new passport.

3. Proof of Identification

  • Valid State of Texas ID
  • Color copy of front and back of ID (single-sided)

4. Photo

  • A 2x2 photo with white background, no hats, glasses, etc. More information can be found at here.

Note: You may choose to take your passport photo on site at Texas State University, if you do not already have one. This service is provided in the Passport Services Office.

5. Payment

  • NOTICE: Kindly be aware that the Execution Fee/Acceptance Fee will be paid to Texas State University with card only during the appointment.
  • NOTICE: Kindly be aware that the Application Fee will be paid to U.S. Department of State via check or money order. You must bring a check or money order to your appointment.
