Form Instructions

Please complete this form to formally request that the department office prepare your ACS final exams.

Complete a separate form for each course section for which you are requesting ACS exams.

Summer II Form Submission Deadline: Friday, July 17, 2026.

If this form is not submitted and you require ACS exams for your final, you (or your grader/volunteer) by the deadline provided at the beginning of the semester will be responsible for coming to the department office to:

Count the number of exams needed.
Check out the exams using the designated department computer and database system.

Important: Any ACS exams that are not prepared or checked out through the department front office staff by submitting a request will be the sole responsibility of the instructor.
