JSCMR Student Pick Up Request Form

Please submit this form to notify Mathworks staff that you will be picking up your child from camp

For security and safety, we ask that you notify your camper's counselor before pick up. You must also check-out your camper with their counselor before leaving and check-in with their counselor when they return to camp.
